If your product touches protected health information in the US, HIPAA applies to you and to every vendor in the path.

Business Associate Agreements. Every service that stores, processes, or transmits PHI needs a signed BAA — hosting, database, video, email, SMS, error tracking, analytics. This is where teams get caught: an error-tracking tool capturing a request body with patient data, or a transactional email service with appointment details in the subject line.

Access controls and audit logs. Every access to a record needs to be attributable and logged. Retrofitting audit logging is expensive; build it with the schema.

Encryption at rest and in transit, which any competent stack gives you, plus key management you can actually describe in a security review.

Buy the video, don't build it

WebRTC is deceptively hard — NAT traversal, TURN servers, reconnection, bandwidth adaptation, and mobile background behaviour. Use a provider that signs a BAA. Twilio Video, Daily, Vonage, and Agora all serve healthcare.

What matters when choosing: BAA availability, recording controls (and whether you want recording at all — it creates retention obligations), waiting-room support, and graceful degradation to audio when bandwidth drops. Patients on poor connections are the normal case, not the edge case.
